In order to change your name on the banking relationship, we kindly ask you to visit a branch and bring the following documentation to verify your name change with you:

• New ID with new and old names
• New ID with the new name only and an official confirmation of name change (such as family register, marriage certificate, court order) or
• Old ID with an official confirmation of name change (such as family register, marriage certificate, court order)

You can easily change your residential address via online banking.

Alternatively, you can initiate the change by sending
us the completed and signed form to the following address:

You can request a power of attorney by phone or contact form. Before we can set this up, you must have the following information available on the authorized representative:

• First and last name
• Date of birth and nationality
• Address and country of domicile

We will then send you the power of attorney by post for your signature. The power of attorney will take effect once we have received the fully completed, signed document.

You can easily change your telephone number via Online Banking. To do so, click here. Alternatively, after logging in you can click on the «User information» button at the top right, and then select «Personal Data». You can then change your telephone numbers by selecting the arrow at right.

If you are already registered with Credit Suisse for another client relationship or if there is a signature registry for your authority in which you are included, please send us a copy of the deed of appointment and an indication of which relationship you are already on file for. We would then be happy to send you the documents requested.

If you are not yet registered with us, you will need the following documents:

• Deed of appointment (copy)
• Completed and signed specimen signature (PDF download)
• You can request online banking via the following link once we have registered your signature. Please select the first option, "for myself."
• Authenticated ID copy or reference relationship for which you are already on file.
